Adoption of revised disability proformas for the recruitment of Persons with Disabilities (PwD).
• All Postal Circles, Training Centres, and Directorates are directed to adopt the revised Form V (for cases of Single Disability) and Form VI (for cases of Multiple Disabilities).
• These revised forms, prescribed under the RPwD (Amendment) Rules, 2024, are intended to standardize and enhance the process of issuing disability certificates and Unique Disability Identity (UDID) cards.
• The revised proformas must be utilized specifically while recruiting PwD candidates to ensure consistency and to prevent the denial of rightful benefits due to outdated documentation.
• The order forwards a previous Office Memorandum (dated January 14, 2026) from the Department of Empowerment of Persons with Disabilities (DEPwD) regarding these updates.
