176. What is the existing provisions on supply of Duplicate copy of Service book to Government Servants?
For new recruits, within one month of the date of appointment, the second copy of the service Book should be given to the Government servant for his safe custody. In January each year, he shall hand over his copy to his office for updation. The office shall update and return it within 30 days. In case of loss of service book, it shall be replaced on payment of a sum of Rs. 500/-.

177. What is the time limit prescribed for seeking alternation of date of Birth?
Under Note 6 to FR 56, it should be made within five years of his entry into Government service establishing the bonafide mistake occurred.
No belated request should be considered vide DOPT OM No. F 19-17/2/92-Estt (A) dated 19.05.1993.
178. What is the procedure for change of name of female employee on account of marriage etc.?
She will submit an application requesting for the change in the name with details about addition/change in surname or deletion in case of divorce/death etc. On receipt application, the Department will affect the change in all circle records.
